Fortive CEO Calls Hospital Unit's Margin Squeeze A Deliberate 'Year Of Setting The Stage'

Fortive CEO Olumide Soroye told analysts on the company's third-quarter earnings call Wednesday that margin pressure in its Advanced Healthcare Solutions unit comes from "deliberate and time-bounded investments," not a structural problem, as segment adjusted EBITDA fell 5% to $88 million.

A segment in investment mode

Fortive, which makes instruments, software and services for workplace safety and productivity and for healthcare, reports Advanced Healthcare Solutions as one of two segments. Its brands include Advanced Sterilization Products (ASP) and Provation.

CFO Mark Okerstrom said the segment's adjusted EBITDA margin fell 230 basis points from a year earlier to 26%, though it was essentially flat from the prior quarter. He attributed the drop to growth investments and to lapping discrete cost benefits booked in last year's third quarter, a headwind of roughly 100 basis points. Revenue rose 3.7% to $340 million, or 3.1% on a core basis.

Adjusted gross margin in the segment was 57.5%, down 90 basis points on product mix, led by faster growth in lower-margin consumables in Asia and Latin America.

Soroye's case for 2026

A JPMorgan analyst noted that even without the one-time headwind, segment margins were still down. Soroye pointed to year-to-date core growth of 4.5% and said the work this year is to set up the unit "for a great run of multi-year acceleration in growth and margin expansion."

The spending goes to growing the installed base to power ASP's razor-and-blade model, adding sales capacity to win share with ambulatory surgical centers, and funding innovation. "View this year very much as a year of setting the stage for the success of this segment," he said.

Asked by a Seaport Research Partners analyst about the year-ago items, Okerstrom said they were largely incentive compensation and capitalized software development, mostly landing in healthcare.

Raised outlook, fourth-quarter drag

Company-wide, Fortive raised full-year adjusted earnings per share guidance to $3.10 to $3.14. Okerstrom said adjusted EBITDA margin expansion should land around the midpoint of the 50 to 100 basis point range.

Fourth quarter has four fewer selling days, which Okerstrom said will cut core growth by 300 basis points in healthcare and 100 in Intelligent Operating Solutions. Soroye said healthcare hospital demand is stable to improving in the U.S. and strong abroad.

Okerstrom said that, with full-year margin expansion at about the midpoint, listeners could back into what it implies for the fourth quarter.
