Cumberland Will File Duchenne Heart Drug Without A Phase 3 Trial, Though The FDA Raised Doubts
The drugmaker plans to seek traditional approval of ifetroban using its Phase 2 study, after the agency questioned the data package.
Cumberland Pharmaceuticals, a specialty drugmaker, plans to submit a New Drug Application for ifetroban, its treatment for the heart disease that comes with Duchenne muscular dystrophy, using a Phase 2 trial as the pivotal study. The FDA had suggested a randomized Phase 3 trial and, at a pre-NDA meeting, voiced concern the data may not meet the bar for approval, the company said in an 8-K filed Monday.
The FDA's doubts, in stages
After an End-of-Phase 2 meeting in September 2025 and a Type C meeting in January 2026, the FDA suggested a Phase 3, randomized, placebo-controlled trial. It also questioned the suitability of the ejection-fraction endpoint, comparator issues and how the statistical analysis plan was developed.
Cumberland responded with more efficacy data, natural history data from a second DMD patient registry and documentation of the analysis plan, then requested the pre-NDA meeting. There, the FDA expressed concern about whether the package would show the substantial evidence of efficacy needed for approval, and asked for additional analysis and supporting information.
The agency then granted Cumberland's request for rolling review, which lets finished sections go to the FDA before the full application is complete. The board approved proceeding, citing the urgency of the unmet medical need. The company cautioned there is no assurance the FDA will accept the filing or approve the drug.
What the Phase 2 data show
In the 12-month FIGHT DMD trial, high-dose ifetroban produced a statistically significant 5.4% absolute improvement in left ventricular ejection fraction (p=0.002). The comparison group combined placebo patients with propensity-score-matched natural history patients. Cardiac damage markers NT-proBNP and troponin I also fell in the high-dose group.
Through up to 36 months of treatment, the company reported no treatment-related serious adverse events. The submission will include 24- and 36-month cardiac MRI results from the open-label extension, which are still being analyzed, plus a safety database of more than 1,400 subjects across about 30 ifetroban studies.
The company and the stock
Cumberland sells branded drugs including Acetadote, Caldolor and Kristalose, mainly to US wholesalers, and is also testing ifetroban in other conditions. Ifetroban is a once-daily oral drug with Orphan Drug, Rare Pediatric Disease and Fast Track designations for this indication, and the FDA has conditionally accepted DYSCORBAN as its proprietary name.
Cumberland shares fell about 49% to $4.35 at Monday's close, from $8.49 in the prior session, a drop that came before the filing was accepted at 4:40 p.m. ET.
Next up: whether the FDA accepts the application for filing, and the 24- and 36-month MRI results headed into the submission.
