[Kiniksa Pharmaceuticals](https://www.bullished.co/tag/knsa/), which develops and sells heart-focused therapies, [raised its 2026 Arcalyst sales forecast](https://www.sec.gov/Archives/edgar/data/0001730430/000173043026000037/knsa-ex99d1.htm?ref=bullished.co) to $1.045 billion to $1.050 billion on Tuesday, from $980 million to $995 million. The move followed third-quarter revenue of $286.0 million, about 58% above a year earlier, and net income of $47.3 million.

## Profit more than doubles on Arcalyst

Arcalyst, Kiniksa's only commercial product, brought in $286.0 million in the third quarter, against $180.9 million in the third quarter of 2025, the company said in its [earnings release](https://www.globenewswire.com/news-release/2026/10/27/3388879/0/en/kiniksa-pharmaceuticals-reports-third-quarter-2026-financial-results-and-recent-portfolio-execution.html?ref=bullished.co). For the first nine months, revenue was $743.8 million versus $475.4 million a year earlier.

Net income was $47.3 million, or $0.56 per diluted share, compared with $18.4 million, or $0.23 per diluted share, a year earlier. Operating expenses rose to [$231.7 million](https://www.globenewswire.com/news-release/2026/10/27/3388879/0/en/kiniksa-pharmaceuticals-reports-third-quarter-2026-financial-results-and-recent-portfolio-execution.html?ref=bullished.co) from $156.8 million, led by collaboration expenses of $105.8 million, up from $63.3 million. Those costs are driven mainly by Arcalyst's profitability, the company said.

Research and development spending was $32.8 million, up from $24.2 million on more KPL-387 trial activity, and selling and administrative costs were $65.4 million, up from $49.1 million.

CEO Sanj K. Patel said strong commercial execution is positioning Arcalyst "to achieve blockbuster status this year."

## A weekly shot for recurrent pericarditis

Arcalyst is a weekly injection approved in the U.S. for recurrent pericarditis, a painful inflammation of the sac around the heart, and for two rare inflammatory conditions, CAPS and DIRA. Kiniksa licenses it from Regeneron, which discovered the drug.

About 23% of the 14,000 annual multiple-recurrence patients were actively on Arcalyst at the end of the third quarter. More than 5,450 prescribers have written prescriptions for recurrent pericarditis since launch, and average therapy duration was about 3 years.

## Cash, spending plans and the pipeline

Kiniksa held $568.8 million in cash, cash equivalents and short-term investments with no debt as of September 30\. It expects its operating plan to stay cash flow positive for the year, with 2026 research spending of $160 million to $170 million and selling and administrative spending of $270 million to $280 million.

The company is enrolling patients in PASTORALE, the pivotal late-stage trial of KPL-387, a monthly-injection antibody for recurrent pericarditis. Data are expected in the second half of 2027, and Kiniksa hopes to reach patients in the 2028/2029 timeframe. It also expects to start a first-in-human trial of KPL-1161, a preclinical antibody with a target of quarterly dosing, by the end of 2026.

The next hard date is the second half of 2027, when Kiniksa expects the PASTORALE readout on KPL-387.
