# Arcus Kidney Cancer Drug Casdatifan Hits 14.5-Month Median PFS With Cabozantinib In Second Line

[Arcus Biosciences](https://www.bullished.co/tag/rcus/) said casdatifan paired with cabozantinib kept patients free of disease progression for a median of [14.5 months](https://www.sec.gov/Archives/edgar/data/0001724521/000172452126000047/rcus-20261020.htm?ref=bullished.co) in second-line clear cell renal cell carcinoma, a kidney cancer type. The figure, from 43 patients, comes from an [8-K](https://www.bullished.co/learn/8-k-items-explained/) filed Tuesday covering updated ARC-20 study data with a September 4, 2026 cutoff.

## Second line: the strongest numbers

In the cabozantinib cohort, median follow-up was [21.3 months](https://www.sec.gov/Archives/edgar/data/0001724521/000172452126000047/rcus-20261020.htm?ref=bullished.co). The confirmed response rate was 44%, primary progression (progression at or before the first scan) was 5%, and 18-month overall survival was 77%.

The filing says efficacy was similar whether or not patients had received a prior TKI (tyrosine kinase inhibitor) therapy. It adds that TKI-pretreated patients are about 60% of second-line patients in current practice and get diminished benefit from later TKIs. No unexpected safety signals appeared among 44 patients.

## First line and late line

In first-line, TKI-free treatment, casdatifan plus the anti-PD-1 drug zimberelimab (30 patients, 11.5 months of median follow-up) showed progression-free survival of 86% at six months and 69% at 12 months. The confirmed response rate was 50% and primary progression was 6.7%.

A triple combination adding ipilimumab had only three months of median follow-up. One of 24 patients (4%) who reached a first scan had primary progression. Among 31 patients dosed, no unexpected safety signals were seen, and serious immune-related side effects were generally consistent with ipilimumab plus nivolumab in frequency, severity and timing.

For late-line casdatifan alone, four pooled cohorts totaling 121 patients had 28 months of median follow-up. Median overall survival was 35 months, median progression-free survival was 13 months and the confirmed response rate was 36%. Results were similar after one, two or three prior TKI-containing treatments.

## What to keep in mind

The triple combination data are early, and the filing reports no comparison against other treatments. Each cohort is a separate group of patients with its own follow-up length, so the figures are not like-for-like. The company said safety data in each combination matched what was seen in other cohorts or earlier studies of each drug.
